I'm so glad I got these just checking box is great. Wish came of on sticky thing like note pad . From PJ's: I inquired about the sticky pads. They were going to be twice the cost, so I opted out. LOL

theres no way to hold them together once you open them...they just fly around everywhere :( From PJ's: You're kidding right? Have you tried a rubber band?

This form sure does save a lot of time by not having to stop and write a note to my customers. I think it covers everything needed to say by just checking the appropriate box. Great idea and cheap!
